Our NDIS Support Coordination service is designed to simplify the system, help you understand your plan, and connect you with the right supports. Whether you’re new to the NDIS, transitioning from one plan to another, or simply need help making the most of your funding, My Capacity is here to walk beside you.

What Is Support Coordination?

Support Coordination is an NDIS-funded service that helps participants:

  • Understand their NDIS plan
  • Connect with service providers and community supports
  • Coordinate services across different areas of their life
  • Build the skills and confidence to manage their own supports over time

At My Capacity, we offer Support Coordination Level 2 and can assist with Specialist Support Coordination (Level 3) for participants with more complex needs.

Who Can Access Support Coordination?

Support Coordination is not automatically included in every NDIS plan. It is typically funded for participants who:

  • Are new to the NDIS or receiving their first plan
  • Are managing complex or multiple support needs
  • Require assistance due to language, cognitive, or psychosocial barriers
  • Are going through a significant life transition, such as leaving hospital, changing living arrangements, or navigating legal/guardianship decisions

  • Identify your goals and what supports can help you achieve them

  • Recommend local providers that suit your preferences and needs
  • Arrange meet-and-greets or service trials to help you make informed decisions
  • Ensure services are inclusive, culturally appropriate, and accessible
  1. Coordinating and Managing Supports
  • Help you schedule appointments and coordinate across providers
  • Work with health professionals, support workers, housing services, and more
  • Monitor progress and support you to adjust your services as needed
  1. Building Your Capacity
  • Support you to make decisions with confidence
  • Teach you how to self-manage or direct your own supports over time
  • Provide tools, tips, and knowledge to take charge of your plan
  1. Crisis and Change Support
  • Assist during periods of change, such as hospital discharge or breakdown in care
  • Coordinate urgent services and liaise with government agencies (e.g., Centrelink, SACAT, SA Health)
  • Advocate for your rights and ensure your voice is heard in decision-making
