What Is Specialist Support Coordination?

Specialist Support Coordination is the highest level of coordination of supports funded through the NDIS. It is intended for participants who require a high level of support due to:

  • Complex health or disability needs
  • Risk of harm, homelessness, or exploitation
  • Involvement with multiple systems (e.g., justice, child protection, mental health)
  • Situations requiring urgent intervention and stabilisation
  • Coordination across various service providers or restrictive environments

This service focuses on reducing complexity in the support environment, helping to manage crises, and ensuring consistent delivery of services—especially where systems are not working well together.

Who Can Benefit from Specialist Support Coordination?

Our Specialist Support Coordination service is ideal for:

Support Coordination is not automatically included in every NDIS plan. It is typically funded for participants who:

  • Are new to the NDIS or receiving their first plan
  • Are managing complex or multiple support needs
  • Require assistance due to language, cognitive, or psychosocial barriers
  • Are going through a significant life transition, such as leaving hospital, changing living arrangements, or navigating legal/guardianship decisions

Key Roles and Responsibilities of a Specialist Support Coordinator

At My Capacity, our Specialist Support Coordinators are skilled professionals with deep knowledge of complex support systems. They help you:

  • Understand and Implement Your NDIS Plan: We make your plan meaningful by translating it into clear, practical steps. We help you understand your budget, goals, and funded supports so you can make informed decisions.
  • Coordinate and Integrate Services: We work across health, mental health, housing, justice, education, and community services to ensure your supports work together—rather than in silos. This includes attending case conferences, coordinating multidisciplinary teams, and negotiating service agreements.
  • Manage and Respond to Crisis: When things go wrong—or are already going wrong—we step in to stabilise the situation. From homelessness prevention to mental health episodes, we act fast to link you with emergency supports, advocate for your rights, and create crisis plans that help protect your safety.
  • Advocate Across Systems: We liaise with hospitals, justice systems, government bodies, housing providers, and more to ensure your voice is heard. We understand how systems work—and where they fall short. We’re here to bridge gaps and hold systems accountable.

Our Process: What to Expect

We make the process of engaging with Specialist Support Coordination simple, respectful, and transparent:

Initial Meeting and Planning

We meet with you (and your trusted supporters) to learn about your situation, goals, and current challenges. This can happen in your home, a hospital, a neutral space—or wherever is most comfortable for you.

Support Mapping and Prioritisation

We create a practical, prioritised plan to stabilise urgent issues, coordinate needed services, and build capacity. We also identify potential risks and create contingency strategies.

Ongoing Coordination and Advocacy

We regularly check in, attend service meetings, advocate on your behalf, and adjust your support plan as needed. Our goal is always to move toward long-term sustainability and improved quality of life.

Monitoring and Review

We support you during NDIS plan reviews, report on outcomes, and help ensure your plan continues to meet your needs. We also gather feedback to improve your experience.
