What Is Specialist Support Coordination?
Specialist Support Coordination is the highest level of coordination of supports funded through the NDIS. It is intended for participants who require a high level of support due to:
- Complex health or disability needs
- Risk of harm, homelessness, or exploitation
- Involvement with multiple systems (e.g., justice, child protection, mental health)
- Situations requiring urgent intervention and stabilisation
- Coordination across various service providers or restrictive environments
This service focuses on reducing complexity in the support environment, helping to manage crises, and ensuring consistent delivery of services—especially where systems are not working well together.
At My Capacity, we go beyond basic coordination. We actively resolve barriers, advocate alongside participants, and work collaboratively with stakeholders to create a clear path forward.
Who Can Benefit from Specialist Support Coordination?
Our Specialist Support Coordination service is ideal for:
- Participants with high-risk needs or vulnerabilities
- Individuals with dual diagnoses or psychosocial disabilities
- People transitioning from hospital, incarceration, or out-of-home care
- NDIS participants subject to restrictive practices or guardianship orders
- Those at risk of losing their housing or existing support networks
- Participants experiencing sudden or ongoing crises
Our role is to bring clarity, stability, and momentum during difficult times—and to ensure that every participant is treated with dignity, respect, and care.
